A patient, parents of minor patients, or caregivers acting on the behalf of a patient (i.e.Healthcare Power of Attorney), may inspect and/or obtain a copy of their medical records, or have copies of medical records sent to another provider or facility.

Patients looking to obtain a hard copy of their medical records can download the “Release of Information” form below and fax the completed form to 903.731.1183.
- Photo Identification is required.
- Complete and sign the release of information authorization form.
